We are looking forward to meeting you and welcoming you to our office.

Melanie is our Treatment Coordinator. She will book your first appointment with us and answer any questions you may have about our office. As most of our patients are referrals from other patients, there is a link on our front page which will give you detailed instructions to our office.

If you would be so kind as to fill out these information sheets it will expedite the paperwork on your first visit. You may fill them out online and send back to us or print them and either fax or bring in with you.

If you have insurance(s) we require completion of the insurance information form. This will allow us to preauthorize treatment and submit claims more efficiently.
